Mission
THE CORPORATION'S MISSION IS TO OWN AND OPERATE AN ACCOUNTABLE CARE ORGANIZATION THAT PARTICIPATES IN VARIOUS ACCOUNTABLE CARE MODELS, INCLUDING THE NEXT GENERATION ACO MODEL (ACO), AND OTHER ACCOUNTABLE CARE ORGANIZATION ARRANGEMENTS. THE INITIATIVE'S AIM IS TO COLLABORATE AND INTEGRATE/COORDINATE CARE FOR MEDICARE FEE-FOR-SERVICE PATIENTS IN A WAY THAT IMPROVES OUTCOMES FOR THE COVERED POPULATION.
