THIS IS AN EMPLOYEE BENEFIT PLAN IN WHICH THE EMPLOYER AND THE EMPLOYEE SHARE THE COST OF THE SELF FUNDED HEALTH INSURANCE PLAN. THE PLAN IS OPEN TO ALL FULL TIME EMPLOYEES THAT WORK AT LEAST 20 HOURS PER WEEK.
