IS TO ESTABLISH, OPERATE AND MAINTAIN A LICENSED HOME CARE SERVICES AGENCY POSSESSING A VALID OPERATING LICENSE ISSUED UNDER ARTICLE 36 OF THE PUBLIC HEALTH LAW. SERVICES TO BE PROVIDED TO CLIENTS IN THE COMMUNITY INCLUDE, BUT ARE NOT LIMITED TO, COMPANION SERVICES, PERSONAL CARE AIDE SERVICES, HOME HEALTH AIDE SERVICES AND SKILLED NURSING AND THERAPY SERVICES.
