TO EMPOWER ADULTS AND CHILDREN LIVING IN LOUISIANA WHO HAVE MENTAL, PHYSICAL, BEHAVIORAL, AND OTHER CHALLENGES TO IMPROVE THE QUALITY OF THEIR LIVES AND TO LIVE AS INDEPENDENTLY AS POSSIBLE IN THEIR COMMUNITIES BY FACILITATING PERSONAL CARE AND FOCUSING ON QUALITY OUTCOMES RESULTING IN OVER 2,400 CONSUMERS BEING SERVED DURING THE PAST YEAR.
