THE PLAN PROVIDES SHORT-TERM DISABILITY, DENTAL, DRUG, OPTICAL, DEATH, ACCIDENTAL DEATH AND DISMEMBERMENT, AND CERTAIN OTHER BENEFITS FOR ELIGIBLE PARTICIPANTS. A PARTICIPANT IS ELIGIBLE IN THE PLAN IF HE OR SHE IS ACTIVELY WORKING FOR THE TOWN AND THE TOWN IS MAKING PAYMENTS TO THE PLAN ON THEIR BEHALF.
