THE ORGANIZATION IS A COLLECTIVELY BARGAINED MULTIEMPLOYER PLAN THAT PROVIDES HEALTH & WELFARE BENEFITS TO ELIGIBLE PARTICIPANTS IN ACCORDANCE WITH THE TERMS SET FORTH IN THE TRUST AGREEMENTS, THE RULES & REGULATIONS OF THE PLAN AND THE APPLICABLE COLLECTIVE BARGAINING AGREEMENT.
