Community Care of the Lower Cape Fear I

Community Care of the Lower Cape Fear, Inc. (CCLCF) seeks to improve health outcomes for eligible Medicare, Medicaid, uninsured, and privately insured enrollees in southeastern North Carolina including New Hanover, Pender, Brunswick, Columbus, Onslow, and Bladen counties. Equipped with 20 years of population health and care management experience, the Interdisciplinary Care Team works with Prepaid Health Plans, ACOs, practices, and providers to improve patient outcomes and experiences, while reducing costs in the communities it serves.
