THE MISSION OF THE HEALTH CENTER IS TO PROVIDE COMPREHENSIVE CARE & OB SERVICES TO THE MEDICALLY INDIGENT AND UNDERSERVED RESIDENTS FROM NORTHWESTERN SUFFOLK COUNTY. THE CENTER SERVES AS THE ENTRY POINT INTO THE HEALTH SYSTEM FOR INDIVIDUALS AND FAMILIES SEEKING SUCH CARE. EVERY PATIENT WILL BE PROVIDED COMPREHENSIVE AND CONTINUOUS PREVENTIVE CARE, PRIMARY DIAGNOSTIC AND TREATMENT SERVICES AND EFFECTIVE COORDINATION OF CARE WITH SPECIALISTS AND SERVICES IN THE LARGER HEALTHCARE SYSTEM.
