OPEN HEARTS PROVIDES COMPREHENSIVE MENTAL HEALTH SERVICES IN COORDINATION WITH MEDICAL PROVIDERS. THE SERVICES ARE MEDICALLY NECESSARY WITH SPECIFIC DIAGNOSIS AND TREATMENT TO REDUCE SYMPTOMS, PREVENT ILLNESS, AND INCREASE QUALITY OF LIFE. OUR INTEGRATED, HOLISTIC MODEL IS BUILT ON THE PREMISE THAT HEALING INVOLVES THE BODY AND MIND, IN THE CONTEXT OF THE ENVIRONMENT AND SOCIAL DETERMINANTS OF HEALTH. WE ACCOMPLISH THIS THROUGH TEAM BASED CARE, SPECIFIC HEALTH GOALS AND LEADING WITH PERSONALIZED CARE THAT PRODUCES RESULTS.
