CRITICAL SERVICES INCLUDE MEALS ON WHEELS, ADULT DAY CARE HEALTH PROGRAM, GERIATRIC SPECIALTY PRIMARY MEDICAL CARE, CASE MANAGEMENT, AND CAREGIVER SUPPORT.
