THE VOLUNTARY EMPLOYMENT BENEFIT ASSOCIATION TRUST HAS BEEN ESTABLISHED TO PROVIDE HEALTH AND DENTAL BENEFITS TO THE RETIRED FORMER EMPLOYEES OF UNIVERSITY MEDICAL ASSOCIATES, CAROLINA PRIMARY CARE PHYSICIANS, PA, MUSC HEALTH PARTNERS, AND CAROLINA HEALTH MANAGEMENT SERVICES IN ACCORDANCE WITH THE PLAN PROVISIONS.
