TO ENSURE PEOPLE WITH DISABILITIES HAVE COMPLETE ACCESS TO THE COMMUNITY IN WHICH THEY CHOOSE TO LIVE. WE DO THIS THROUGH ADVOCACY, INFORMATION AND REFERRAL, INDEPENDENT LIVING SKILLS TRAINING, PEER SUPPORT, RECYCLING OF MEDICAL EQUIPMENT AND ASSISTING INDIVIDUALS WHO ARE TRANSITIONONG FROM A NURSING FACILITY TO A COMMUNITY SETTING.
