THE ASSIST CENTER PROVIDES NON-MEDICAL, SHORT-TERM LODGING FOR INDEPENDENT INDIVIDUALS WHOSE MEDICAL PROVIDERS BELIEVE CLOSE ACCESS TO MEDICAL CARE WILL INCREASE THE INDIVIDUAL'S ABILITY TO IMPROVE THEIR HEALTH AND INDEPENDENCE. THE ASSIST CENTER ALSO PROVIDES TRANSPORTATION SERVICES AND SERVES TO CONNECT PATIENTS WITH COMMUNITY RESOURCES.
