WORKING IN LONG-TERM PARTNERSHIP WITH LOCAL JURISDICTIONS, CHC PROVIDES AFFORDABLE RENTAL HOUSING AND WRAP-AROUND SUPPORTS TO FAMILIES AND INDIVIDUALS EXPERIENCING HOMELESSNESS OR WHO ARE UNABLE TO MAINTAIN STABLE HOUSING DUE TO ECONOMIC OR OTHER HARDSHIPS.
