WORKING WITH PRIMARY CARE PHYSICIANS, THE ORGANIZATION PROVIDES A TEAM OF EXPERTS THAT INCLUDES A MEDICAL DIRECTOR, NURSES, SOCIAL WORKERS, COUNSELORS, VOLUNTEERS AND HOME HEALTH AIDES.
