Grants are made according to trust agreeement dated December 18, 1967,as amended. Keybank, as trustee, makes distributions as follows: 50%to Lakewood United Methodist Church, 30% to The Three Arches Foundation, FKA Lakewood Hospital, 10%to Lakewood Kiwanis, 5% to Lakewood Christian Church, and 5% to theBeck Center. Under the Ohio Institutional Trust Funds Act of 1999, thebeneficiaries have chosen to receive the maximum of income based onthe greater of 5% of AFV or traditional income. Keybank maintains aresponsiveness to the needs and activities of each beneficiaryorganization in the attainment of its mission. Please note thecharities have a right to remove the Trustee, the authority to appointco-trustees, and the Trustee has the authority to amend the documentto insure the continuation to qualify as a 509(a)3.
