IS ORGANIZED TO DESIGN AND DEPLOY A WELLNESS AND WELL-BEING PLATFORM COMPRISED OF PROGRAMS AND ACTIVITIES FOCUSING ON PREVENTION, DETECTION, AND INTERVENTION OF CHRONIC DISEASE MANAGEMENT THROUGH HEALTH SCREENINGS, EDUCATION, COACHING, AND DEFINED PRIMARY CARE COORDINATION AND CLINICAL STRATEGIES IN ORDER TO REDUCE HEALTH PLAN EXPENSES WHILE IMPROVING HEALTH AND WELL-BEING OF THE PROGRAM POPULATION.
