RESPECTIVELY AND PROACTIVELY FACILITATES SERVICES, SUPPORTS, AND SOLUTIONS; AND TO ADVOCATE FOR INDIVIDUALS AND FAMILIES THAT IT SUPPORTS; TO COORDINATE WITH PROVIDERS AND COMMUNITY PARTNERS AND TO ASSIST PEOPLE IN ACHIEVING SELF DETERMINATION AND MAKING QUALITY OF LIFE CHOICES. SERVICES ARE PROVIDED THROUGH A CONTRACT WITH THE STATE OF OREGON, DEPARTMENT OF HUMAN SERVICES. CONTRACTS ARE RENEWABLE EVERY TWO YEARS.
