THE MISSION OF DUFFY HEALTH CENTER IS TO IMPROVE THE QUALITY OF LIFE FOR PERSONS WHO ARE HOMELESS OR AT RISK OF HOMELESSNESS ON CAPE COD THROUGH COMMUNITY COLLABORATIONS AND THE PROVISION OF INTEGRATED MEDICAL, MENTAL HEALTH, ADDICTION TREATMENT, CASE MANAGEMENT AND HOUSING SUPPORT SERVICES.
