THE PRIMARY CARE COALITION IS A 501(C)(3) THAT ENVISIONS A VIBRANT COMMUNITY THAT SUPPORTS ALL PEOPLE IN ACHIEVING HEALTHY LIVES. OUR MISSION IS TO IMPROVE THE HEALTH OF VULNERABLE PEOPLE AND FAMILIES BY BUILDING PARTNERSHIPS AND STRENGTHENING SYSTEMS.
