IN EXCHANGE FOR A MONTHLY FEE, THE PROGRAM PROVIDES BENEFITS FOR EACH LEVEL OF CARE IN HEALTH & REHABILITATION, ASSISTED LIVING, AND MEMORY SUPPORT TO SUPPLEMENT THE COST OF HIGHER LEVELS OF CARE.
