NHPCO'S MISSION CALLS US TO LEAD AND MOBILIZE THE TRANSFORMATION OF CARE DELIVERY TO ENSURE EQUITABLE ACCESS TO HIGH QUALITY INTERDISCIPLINARY, PERSON-CENTERED CARE FOR THOSE LIVING WITH SERIOUS ILLNESS. OUR STRATEGIC PLAN CALLS FOR NHPCO TO (1) SET THE GOLD STANDARD FOR HOSPICE AND PALLIATIVE CARE, (2) SERVE AS THE UNIFIED VOICE FOR THE HOSPICE AND PALLIATIVE CARE COMMUNITY, (3), ADVANCE INTERDISCIPLINARY, PATIENT-CENTERED, GOAL-ORIENTED CARE, AND (4) EMPOWER SERIOUSLY ILL INDIVIDUALS TO ACCESS INTERDISCIPLINARY CARE. TO ACHIEVE THESE GOALS, WE WORK IN TANDEM WITH OUR ALLIED ORGANIZATIONS, THE HOSPICE ACTION NETWORK AND THE NATIONAL HOSPICE FOUNDATION.
