TO PROVIDE PARTICIPANTS THE CARE AND TREATMENT THEY NEED TO REMAIN INDEPENDENT AND IN THEIR HOME IN THE COMMUNITY FOR AS LONG AS POSSIBLE. MOST PARTICIPANTS ARE LOW-INCOME.
