A NATIONALLY RECOGNIZED PROVIDER OF SPECIALIZED SERVICES FOR CHILDREN WITH COMPLEX HEALTHCARE NEEDS REQUIRING MEDICAL, REHABILITATION, PSYCHOLOGICAL AND DEVELOPMENTAL INTERVENTION. SMH PROVIDES BOTH SUB-ACUTE AND LONG-TERM CARE SERVICES, AS WELL AS A NUMBER OF HIGHLY SPECIALIZED, TECHNOLOGICALLY SOPHISTICATED HOME-AND COMMUNITY-BASED SERVICES. SMH MAKES REFERRALS TO AND WHEN APPROPRIATE, ACCEPTS ADMISSIONS FROM HOSPITALS AND OTHER PROVIDERS THROUGHOUT THE NEW YORK METROPOLITON AREA AS WELL AS FROM ST. MARY'S HOME CARE (A CERTIFIED HOME HEALTH AGENCY FOCUSED ON THE PROVISION OF SERVICES TO CHILDREN AND YOUNG ADULTS WITH SPECIAL NEEDS).
