The Transition Houses mission is to support individuals in reintegrating into the community by providing the resources and skills necessary to become productive members of society. Our programs offer individualized assessments and treatment in mental health, substance abuse, and reentry services, along with housing support through residential facilities and work release centers. We emphasize recovery, stability and long-term self-sufficiency in collaboration with families, support systems, and community partners.
