(LIVING INDEPENDENCE FOR THE ELDERLY). LIFE IS AN INTEGRATED MODEL OF CARE DESIGNED TO ENABLE MEDICARE AND MEDICAID TO SERVE SENIORS THROUGH A SEAMLESS PROCESS OF CONTINUOUS CARE AND SERVICES. THIS PROGRAM ENABLES THE FRAIL NURSING HOME ELIGIBLE LOW INCOME INDIVIDUALS THE OPTION OF CONTINUING TO LIVE IN THE COMMUNITY.LIFE COORDINATES AND PROVIDES ALL NEEDED PREVENTIVE, PRIMARY, ACUTE AND LONG-TERM CARE SERVICES TO ITS PARTICIPANTS THROUGH AN INTERDISCIPLINARY TEAM OF HEALTH CARE PROFESSIONALS THAT EVALUATE, ASSESS AND PROVIDE FOR THE INDIVIDUAL NEEDS OF THE PARTICIPANT AS THEY CHANGE THROUGH VARIOUS STAGES OF AGING OR MEDICAL NEED. DELIVERY OF CARE IS RENDERED THROUGH AN ADULT DAY HEALTH CENTER THAT PROVIDES AN ARRAY OF SERVICES FROM PHYSICIAN CARE TO SOCIAL STIMULATION.
