OUR MEMBERS FORMED CHPA TO ACHIEVE THE TRIPLE AIM AND IMPROVE CARE TRANSITIONS AND BEST PRACTICES IN OUR PATIENT-CENTERED MEDICAL HOME CARE MODEL; AND TO DEVELOP ALTERNATIVE PAYMENT METHODOLOGIES AND PAYMENT REFORM WHICH DECREASE OVERALL HEALTHCARE COSTS AND INCREASE PATIENT SATISFACTION.
