BY ORGANZIING A FORMAL HEALTHCARE PROVIDER COLLABORATIVE, MHNU'S MISSION IS TO IMPROVE THE HEALTH OF MEDICAID RECIPIENTS IN CHICAGO BY ENHANCING CARE COORDINATION AND QUALITY, IMPROVING ACCESS AND REDUCING FRAGMENTATION AND COST, ALL WHILE REINFORCING THE MEDICAL HOME. MHNU'S GOAL IS TO SERVE AS A MODEL FOR COLLABORATIVE AND PATIENT-CENTERED CARE.
