WASHINGTON AND JANE SMITH COMMUNITY - BEVERLY WAS INCORPORATED FOR THE CHARITABLE PURPOSE OF PROVIDING INDEPENDENT LIVING, ASSISTED LIVING, AND NURSING CARE SERVICES TO MEN AND WOMEN THROUGH THE OPERATION OF A CONTINUING CARE RETIREMENT COMMUNITY. (SEE SCHEDULE O) LOCATED IN CHICAGO, ILLINOIS. WITHIN THE COMMUNITY, WASHINGTON AND JANE SMITH COMMUNITY - BEVERLY OPERATES A 149 INDEPENDENT LIVING UNITS, 82 BED ASSISTED LIVING, AND 78 BED SKILLED NURSING FACILITY LICENSED BY THE ILLINOIS DEPARTMENT OF PUBLIC HEALTH (IDPH). ALL 100 SKILLED NURSING BEDS ARE CERTIFIED FOR PARTICIPATION IN THE MEDICARE AND MEDICAID PROGRAMS. WASHINGTON AND JANE SMITH COMMUNITY - BEVERLY IS SUBJECT TO PERIODIC REGULATORY INSPECTIONS PERFORMED BY IDPH.
